Efficacy of Clascoterone 1% Cream in Mild to Moderate Acne

Abstract:

Background:
Acne is a multifactorial condition characterized by excess sebum production, epithelial hyperkeratinization, Cutibacterium acnes colonization in the pilosebaceous unit, and inflammation. Circulating and locally synthesized natural ligands, testosterone and dihydrotestosterone (DHT), serve as causative factors in both males and females. Upon binding of DHT, the DHT-androgen receptor complex dimerizes and translocates to the nucleus where it promotes the transcription of genes involved in acne pathogenesis, including proliferation and differentiation of sebocytes, excess sebum production, and inflammatory cytokine production. Clascoterone is a potent antagonist at ARs and competes for androgens in binding to the receptor, thereby inhibiting downstream signalling of ARs that promote acne.
Objective:
To observe the efficacy of Clascoterone 1% cream in treatment of mild to moderate facial acne vulgaris.
Materials and Methods:
This cross sectional observational study was conducted in Department of Dermatology and Venereology, Dhaka Medical College Hospital, Dhaka. Patients of mild to moderate facial acne vulgaris were enrolled for study. Clinical examination and relevant investigation were done meticulously. A total number of 50 patients were selected by purposive sampling technique. Topical Clascoterone 1% cream was prescribed as a monotherapy and patients were followed up on 2nd, 4th, 8th and finally on 12th week to see clinical improvement and effects. All information was recorded in data collection sheet. Data was processed and analysed with the help of computer program SPSS and Microsoft excel.
Results:
The mean age was found 20.4±5.7 years. Male to female ratio was almost 1:3. Baseline mean score for open comedone, closed comedone, papule, pustule and total score was 12.32±1.25, 14.17±1.35, 9.60±2.18, 4.01±1.43 & 40.1±2.85 respectively. Significantly better reduction of acne score for open comedone, closed comedone, papule, pustule and total acne score was noticed from 3rd follow up (p<0.005). At 4th follow up mean score for open comedone, closed comedone, papule, pustule and total score was 0.28±0.22, 1.92±0.65, 0.82±0.29, 0.85±0.33 & 3.87±1.50 respectively.
Conclusion:
Present study concluded that topical Clascoterone 1% therapy was found effective agent in treatment of mild to moderate facial acne vulgaris. Overall outcome was better as a monotherapy, no need for other combination agent.
